Practice Policies

Financial Policy

Payment is due at the time services are rendered and we accept: Cash or check is preferred, we also accept credit card, HSA, or FSA cards.  We an out-of-network provider with insurance and do not accept medicare assignment but we will happily provide you a receipt when asked so that you may submit for reimbursement from your insurance provider. The receipt you will be given is called a "superbill" for those who submit these claims to their insurance companies. If you have Medicare, it is more cost conscious to see a Medicare participating provider, which we are not.

If a check is returned by the bank, you will be responsible for the check return fee (the amount the bank charges us) and the cost of original check amount.

When arriving for your appointments, please be timely as to prevent your appointment from overlapping into the next scheduled appointment time. If you are 10 minutes late for your appointment, you will be asked to reschedule and may be responsible for a missed appointment fee.

Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the doctor's day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment.

Established patients who provide less than 24 hours notice to cancel/reschedule, or miss their chiropractic appointment completely, will be charged a cancellation fee of $30 automatically to the credit card on file. If no active card is on file, this fee is due before your next visit for chiropractic care or a sauna session, or 30 days from the missed appointment, whichever comes first.

New patient appointments will be auto-charged $50 for missed appointments or lack of 24 hour notice for any cancellations or changes to your appointment.

If you will be late for your appointment, please email, call or text us to let us know. If you are more than 10 minutes late, you will be asked to reschedule and may be billed for your full appointment cost.

Sometimes life gets in the way of our plans, and we do understand. Missed appointments (No Call/No Show), late cancellation and appointment change fees will be waived one time only.

Nondiscrimination Policy

It is the policy of Kukenberger Chiropractic to treat all patients without discrimination in regards to race, color, religion, national origin, age, ability, sex, sexual orientation, gender identity or expression (the patient’s preferred gender will be respected, and the patient will be referred to by their name and pronoun of choice). Discrimination or harassment is prohibited and will not be tolerated within the office.

Please note that our building is not ADA accessible. There are 8 steps that must be ascended to reach our practice inside of the building. If this presents a challenge for your condition, please call us so we can try to offer an accommodation.
